Dementia and Alzheimer’s Care

Each person is unique and dementia effects people differently. The specific symptoms that someone with dementia experiences will depend on the part of the brain that is damaged and possibly the type of dementia they have but commonly starts with short-term memory loss.

Companionship and Social Activities

SD Homecare work within in the community, building up contacts and communication within local clubs, so you need not let a lack of confidence, immobility issues, anxieties or anything else stop you.  We can help, we can show you the way, and we can accompany you.

Live–In Care, Sleep-Ins and 24 Hour Care

SD Homecare will visit you to discuss what type of package is most appropriate for you. We will also talk to you (and family and stakeholders if required) about the essential times and types of care, what and when the carer will do the chores and when the breaks will be.

Home from Hospital

We will tailor a care plan that meets your needs both when you arrive home and again when you have settled in, at home. We want to make sure that you are safe and do everything to ensure there are no re-admissions.

Domestic Support and Household Duties

Age-related heath and physical problems may prevent you from doing your household duties or you may have just come out of hospital and need some support to assist you to get back on your feet, or physically you just need some assistance to get things done.

Medication Assistance

You could develop swallowing difficulties and may need medicines in different forms, we may have to adapt foods, (such as thickening liquids and mashing solids so that you can consume them). You may develop short or long-term difficulties and we need to notify the appropriate health professionals.

    We visit you at home (or arrange a video/phone call) to understand your needs, routines, preferences and any risks.
  2. Tailored Care Plan
    We agree what kind of support you want, how often visits will be, and what outcomes you’re aiming for – for example, feeling safer at home, giving a family carer a break, or building confidence after a hospital stay.
  3. Matching You with Carers
    We match you with trained, DBS-checked carers whose experience and personality fit your needs. Where possible, we keep your care team small and consistent, so you see familiar faces.
  4. Ongoing Reviews
    We check how things are going, talk through any changes and adjust your care plan as needed. Care can increase, decrease or change type over time.
